England: ‘Highly variable’ standards of community care
Joint Commission’s review of mental health services
The national review carried out by the Healthcare Commission and the Commission for Social Care Inspection assessed England’s 174 Local Implementation Teams [LITs] on three criteria: the involvement of people who use services; access to appropriate care and treatment [such as talking therapies] and recovery and social inclusion.
Just over half were found to have consistently performed well in all three areas, the rest scored fair or received weak evaluations.
A range of community mental health teams are in place in every area of England, which was cited as a positive step, but the standard of care was found to be highly variable across the regions.
A key finding was that the Care Programme Approach [CPA] is not being implemented in a way that puts people first or offers them enough choice; which the Commission says could compromise safety.
Of particular concern is the fact that people were not properly or routinely involved in decisions about their care and treatment.
Community mental health services should provide services that meet people’s needs without restrictions on their freedom and quality of life. For this to work, the Commission states that people with mental health needs must be active participants at an individual and community level, but this is not happening and must be urgently addressed.
The report concludes that LITs appear to be an effective way to deliver new models of community mental health services, but recommends they must review their practices to ensure they are implementing the CPA to the benefit of mental health service users.
The Healthcare Commission and Commission for Social Care Inspection will use the forthcoming Mental Health Patient Survey and the 2007/8 Annual Health Check to identify the level of improvement.
