Canada: Dementia Service Framework provincial initiative
First person–centred care model for British Columbia
Dementia patients and their families will benefit from a new person–centred care model approach for patients with diseases, such as Alzheimer’s.
The model, which is the first of its kind in Canada, will be initiated with a $1 million provincial grant from the Ministry of Health to the Alzheimers Society of British Columbia. The plan is to orientate health services around the needs of the patient and achieve measurable progress towards a better quality of life for those affected by dementia.
The Alzheimers Society and the Ministry of Health will jointly determine changes in the system with the input of partners and stakeholders, under the Dementia Services Framework, which brings together those who receive, plan and deliver care and services to the population.
